Physician Lead, Geriatric Primary Care

Responsibilities

• Deliver and oversee home-based primary care for medically complex, home-limited patients.
• Provide direct geriatric primary care and coordinate in-home visits, telehealth, and care management.
• Guide and mentor an interdisciplinary clinical team.
• Collaborate with community providers to ensure comprehensive patient care.
• Focus on reducing avoidable hospitalizations through evidence-based, patient-centered approaches.

Qualifications & Skills

• Medical degree with licensure to practice.
• Experience in geriatric or primary care, with a focus on home-based services preferred.
• Strong leadership and mentoring abilities.
• Excellent communication and collaboration skills.
