Physician- Geriatrics & Extended Care- Community Living Center Medical Director

About the role

This position oversees the service, quality, and safety of the Community Living Center (CLC) along with leadership members of the CLC and the organization.

Responsibilities

• Attend daily hospital or CLC huddle to maintain communication with hospital teams
• Assist with CLC Consultation as needed in hospital or from non-VA settings
• Serve as CLC Attending for majority of the year in rotation
• Understand, manage, and report data related to/from CLC (local and national)
• Serve as a member of the GEC Leaders Group (meeting monthly and ad hoc)
• Work closely with Community Living Center Assistant Director, Nurse Manager, Social Worker, MDS Coordinator, QI Nurse, and providers in rotation to ensure efficient and safe daily operations of the nursing home
• Serve as Chair or Co-Chair of CLC Steering Committee
• Lead CLC strategic planning, performance improvement, quality assurance, continuous survey readiness, and committee/work groups in collaboration
• Ensure VERA reimbursement matches the care provider on the CLC each year
• Assist in review, understanding, and revision of CLC policy and VHA guidance
• Assist with response and management of Action Items related to CLC
• Assist with response and management of emergencies as it relates to CLC
• Serve as one local point of contact and subject matter expert as it relates to geriatric care and nursing home care

Qualifications

• Currently employed physician(s) in VA who met the requirements for appointment under the previous qualification standard at the time of their initial appointment are deemed to have met the basic requirements of the occupation.
• Board Certified Preferred Experience: Geriatric Medicine Board Certified, 2 or more years of experience in a geriatric clinical setting, and/or 2 or more years of VA experience.
