About the role
Sutter Medical Group of the Redwoods (SMGR) is recruiting a Board-Certified Geriatrician to join our established Geriatric practice within Sutter Senior Care. This is an exciting opportunity to practice in a team-based, value-driven outpatient setting focused on delivering high-quality care to older adults, particularly those with complex medical and social needs.

- Monday through Friday clinic schedule
- Shared call with physician and APC team
- Emphasis on value-based, panel-oriented care
Key Responsibilities
- Provide comprehensive outpatient care for geriatric patients
- Diagnose and manage chronic and complex conditions (e.g., diabetes, hypertension, heart disease, cognitive disorders)
- Deliver preventive care, screenings, and immunizations tailored to seniors
- Manage polypharmacy and medication safety
- Collaborate with APCs, nurses, social workers, and specialists to deliver coordinated care
- Coordinate referrals and support care transitions across settings
- Educate patients and caregivers on disease management and healthy aging
- Participate in team huddles, quality initiatives, and value-based care programs
Qualifications
- MD or DO; Board Certified in Geriatrics, Internal Medicine, or Family Medicine
- Valid and unrestricted California medical license (or eligibility)
- Experience in outpatient geriatrics or primary care preferred
- Experience in interdisciplinary, team-based care models preferred
- Strong clinical, communication, and patient engagement skills
- Passion for caring for older adults and underserved populations
Pay
Competitive base salary aligned with primary care compensation: $310,000 - $340,000
Potential Incentive bonus up to $50K per Year {tied to quality and performance metrics}
