Responsibilities

• PACE program with interdisciplinary team approach
• Patient panel of approximately 130 patients
• Onsite blood draw and portable X-ray services
• Focus on frail and elderly patient population
• Fully funded by Medicare and Medicaid program

Requirements

• Board-certified or board-eligible positions available
• English and Spanish language skills preferred

Schedule

Monday – Friday 8 am – 5 pm schedule
