Job Description

The Primary Care / CDU Physician at Acclaim Multispecialty Group, a partner with JPS Health Network, provides comprehensive, patient-centered care across both outpatient and observation settings. Clinical responsibilities are equally divided between Medical Home Southwest (outpatient clinic) and the Clinical Decision Unit (CDU).

Essential Duties And Responsibilities

  • Provide comprehensive primary care to an assigned patient panel, including preventive care, acute visits, and chronic disease management.
  • Cook up multidisciplinary care delivered to their panel to optimize patient outcomes.
  • Diagnose, treat, and manage acute and chronic conditions using evidence-based guidelines.
  • Deliver preventive services, including screenings, immunizations, and health education.
  • Review and interpret patient medical records to guide clinical decision-making.
  • Maintain accurate and timely documentation in the electronic medical record (EMR).
  • Participate in care team huddles, population health initiatives, and panel management activities.
  • Serve as a resource to patients, families, and the community, including participation in outreach activities as appropriate.
  • Collaborate with residents, medical students, advanced practice professionals, and staff in a teaching environment.
  • Participate in performance improvement programs, peer review, and departmental initiatives.

Additional Responsibilities

  • Contribute to departmental meetings, committees, and faculty development activities where appropriate.
  • Support institutional goals related to patient experience, access, throughput, and clinical outcomes.
  • Partner and communicate effectively to promote a positive experience for patients, staff, and community stakeholders.

Knowledge, Skills, And Abilities

  • Strong clinical skills in both outpatient primary care and acute care/observation medicine.
  • Ability to effectively balance dual practice environments.
  • Commitment to high-quality, patient-centered care.
  • Strong interpersonal, communication, and teamwork skills.
  • Ability to work effectively in a large public health system.
  • Interest in teaching, mentoring, and academic medicine (preferred).
  • Proficiency in EMR systems (EPIC) and clinical documentation.

Qualifications

  • Doctor of Medicine (MD) or Doctor of Osteopathy (DO) from an accredited institution.
  • Completion of an accredited Family Medicine residency.
  • Experience in both outpatient primary care and hospital-based or observation medicine.
  • Experience or strong interest in graduate medical education.
  • Experience in quality improvement, research, or clinical operations.
  • Current, unrestricted Texas Medical License (or eligibility).
  • Board Certified or Board Eligible in Family Medicine.
