Founded in 1967, The HealthCare Connection (THCC) was Ohio’s first Federally Qualified Health Center (FQHC). About the role

We are seeking a dedicated and experienced Physician to join our clinical team. The Physician provides primary care to patients performing a variety of direct patient services including assessment, diagnosis, implementation of treatment, evaluation, and monitoring the general patient management plan for clinic patients. The physician performs these functions under a current practice agreement in a primary health care setting supporting the vision, mission and core values of the organization.

Responsibilities

  • Provides quality patient care through diagnosis and treatment of illnesses.
  • Obtain developmental, social and family history from patient / parent.
  • Examine patients and diagnose.
  • Order appropriate tests.
  • Determine medication, dosage, and side effects.
  • Communicate and educate patient on their particular situation.
  • Facilitate appropriate referrals as needed.
  • Promote preventative medicine.
  • Obtain developmental history from patients.
  • Order immunization doses / schedule.
  • Provide guidance to parent/patients regarding nutrition, safety, discipline, injury and disease prevention.
  • Document patient visits including medical history, physician exam, diagnoses, and plan of action.
  • Review patient file records including allergies, problems, medication lists, well-child visits, immunization status.
  • Follow-up with patient progress and patient tests.
  • Maintain record of high risk patients.
  • Contact parent/patient for update.
  • Recommend follow-up visits.
  • Review and interpret test results of patients.
  • Sign off on lab reports.
  • Take action regarding abnormal lab results.
  • Serve as supervising Physician to designated Nurse Practitioner(s) as required.
  • Apply principles of the Patient-Centered Medical Home to care coordination roles.

Qualifications

  • Medical Doctor Degree
  • Board Certified
  • 3 years of residency training

Preferred Qualifications

  • Experience in primary care, family medicine, or community health
  • Bilingual skills (if applicable)
  • Familiarity with electronic health record (EHR) systems
