Key Responsibilities

  • Serve as the primary in-house medical provider within assigned facilities
  • Manage care for both short-term rehabilitation and long-term care residents
  • Conduct comprehensive assessments, physical exams, and diagnoses
  • Evaluate and manage acute and chronic medical conditions
  • Prescribe medications and treatment plans as appropriate
  • Complete monthly wellness and stability visits
  • Assess and respond to changes in patient condition
  • Participate in Advanced Care Planning discussions
  • Collaborate with interdisciplinary teams, attending physicians, and medical directors
  • Maintain accurate documentation and CPT coding
  • Coordinate referrals to specialists and additional healthcare services as needed

Qualifications

  • Active and unrestricted Physician Assistant license
  • Current DEA registration
  • Master’s Degree from an accredited Physician Assistant program
  • Strong clinical judgment and communication skills
  • Ability to thrive in a fast-paced healthcare environment
