This role requires a collaborative approach to healthcare, working closely with an integrated care team that includes behavioral health and dental services.
The Physician Assistant/Nurse Practitioner provides comprehensive primary and ambulatory care at Peak Vista Community Health Centers and designated locations. Responsibilities include evaluating, diagnosing, treating, and managing acute and chronic conditions, delivering preventive care, and collaborating with an integrated healthcare team to ensure high-quality, patient-centered care. The provider also maintains timely documentation, participates in quality improvement and community outreach initiatives, and supports Peak Vista's mission to improve access to compassionate, high-quality healthcare.
Responsibilities
- Evaluate, diagnose, treat, and manage acute and chronic conditions.
- Deliver preventive care.
- Collaborate with an integrated healthcare team to ensure high-quality, patient-centered care.
- Maintain timely documentation in a template-based electronic medical record (EMR) system, such as EPIC.
- Participate in quality improvement and community outreach initiatives.
- Manage an average patient volume of approximately 22 patients per day.
Requirements
- Active, unrestricted Colorado medical license.
- Active DEA registration (preferred).
- Experience utilizing or willingness to learn a template-based electronic medical record (EMR) system, such as EPIC.
- Commitment to team-based care and integrated healthcare delivery.

Pay
$117,374 to $144,355/yearly, based on experience. Plus, a $10,000 sign-on bonus.
