Advanced Practice Provider (PA / NP) – Sleep Medicine (Outpatient) Part Time - Days The Advanced Practice Provider (APP) in Sleep Medicine is responsible for the outpatient evaluation and management of patients with sleep-related disorders in collaboration with the attending physician. The APP is committed to delivering high-quality, evidence-based care that improves patient outcomes and enhances access to care. The APP practices within established clinical protocols, professional standards, institutional policies, and collaborative agreements. Responsibilities are performed based on clinical training, experience, specialty standards, and physician oversight. Primary Responsibilities Perform comprehensive evaluations of new and return patients in ambulatory sleep medicine settingsObtain detailed sleep histories and perform focused physical examinationsDiagnose and manage sleep disorders including Obstructive and central sleep apnea Insomnia Narcolepsy and hypersomnia disorders Restless legs syndrome Interpret diagnostic testing, including polysomnography and home sleep studiesDevelop and implement individualized treatment plansInitiate and manage PAP therapies (CPAP, BiPAP, APAP)Prescribe medications and manage ongoing treatment plansDocument H&Ps, progress notes, and follow-up visits in the EMRAssist with transitions of care for hospitalized patients as neededCollaborate with physicians, office staff, and sleep lab personnelCoordinate care with referring providers and other specialtiesCommunicate effectively with patients, families, and interdisciplinary teamsFacilitate timely and appropriate patient care deliveryEducate patients on sleep disorders, treatment options, and sleep hygieneReview diagnostic results and adjust care plans accordinglyPrescribe and refill medicationsSupport adherence to PAP therapy and troubleshoot barriers to complianceAssist in development and implementation of sleep medicine programsParticipate in quality improvement initiatives and outcome monitoringSupport adherence to clinical guidelines and institutional standardsContribute to improving patient outcomes and operational efficiency UNIQUE OR SPECIALIZED ASPECTS OF THE JOB Focus on chronic disease management of sleep disorders Integration of diagnostic technology (sleep studies, PAP data monitoring) High emphasis on patient education and long-term adherence Collaboration across multiple specialties Schedule Part-time, DaysCurrent scheduling for this role is primarily weekdays. Future scheduling needs, including potential weekend coverage or call, may be adjusted based on patient care needs, program growth, and operational priorities.
