Physician Assistant - Home Based Primary Care

About the role

The Physician Assistant (PA) practices in Home Based Primary Care Service (HBPC) to provide a full range of services emphasizing quality, timeliness, and the advancement of the Veterans Affairs (VA) mission. The PA provides advanced clinical practice, consultation, and management.

Responsibilities

• Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice in the patient's home.
• Develops and implements an individualized plan to achieve optimal population health and disease management.
• Demonstrates leadership and works collaboratively with Home Based Primary Care (HBPC) interdisciplinary team members.
• Orders and interprets labs, tests, and diagnostic studies.
• Prescribes pharmacologic and non-pharmacologic interventions.
• Initiates referrals and consults to specialty services.
• Provides education to patients and their families.
• Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes.
• Determines the need for and facilitates patient admission to the VA medical facility or other transitions of care.

Requirements

None beyond the basic requirements. This is the entry level for PAs.

Requires adequate vision to accurately perform the assigned duties, distinguish basic colors and assess patients and the environment safely. The position requires the ability to hear at a level to perform the assigned duties, and the use of hearing aids is permitted.

Qualifications

• Master's degree in any discipline or a bachelor's degree in any discipline with one year of experience as a Physician Assistant ~AND~ Successfully passed the PA National Certifying Exam (PANCE) (or are you in the process to taking the exam)
• Currently hold, or will hold, a full and unrestricted license to practice in a State, Territory, or Commonwealth of the United States or in the District of Columbia.
