Physician Assistant

Responsibilities

• Conduct in-home and virtual health assessments to deliver value-based care.
• Review medical histories, reconcile medications, and order/interpret basic tests.
• Accurately document findings in the EHR system.
• Collaborate with interdisciplinary teams to address care gaps and coordinate follow-up.
• Support quality improvement and risk adjustment programs.

• Passion for improving health outcomes, especially for vulnerable and Medicare/Medicaid populations.
