Physician (7 on/7off schedule)
About the role
Curana Health is seeking a mission-driven Physician to serve in an inpatient comprehensive rehabilitation facility (IRF). This role offers a true 7 on / 7 off block schedule—seven consecutive days on service followed by seven days completely off—providing 26 weeks of protected time per year with a predictable rhythm you can plan a life around. We're looking for physicians who want to focus on high-acuity inpatient rehabilitation medicine while reclaiming the rest of their calendar.
Responsibilities
- Lead the interdisciplinary team in an inpatient comprehensive rehabilitation facility (IRF).
- Manage medically complex patients through their functional recovery.
- Mentor Advanced Practice Providers (APPs) and advocate for patients in your care.
- Complete admission history and physicals, establish rehabilitation goals, and document individualized plans of care within required timeframes.
- Conduct face-to-face visits at least three days per week (daily as clinical acuity requires) to assess progress and adjust care plans.
- Lead weekly interdisciplinary team conferences with PT, OT, speech-language pathology, nursing, case management, and social work to set goals, track progress, and plan discharge.
- Review and sign off on preadmission screenings to confirm rehabilitation appropriateness and IRF medical necessity criteria.
- Manage active comorbidities, medications, pain, and post-acute complications to enable full participation in intensive therapy.
- Partner with APPs to manage daily rounding, complex visits, and after-hours needs while ensuring compliance and clinical excellence.
- Oversee APP performance, serving as a mentor and resource to elevate care delivery across the team.
- Close quality gaps to drive measurable improvements in functional outcomes, length of stay, and discharge to community.
- Coordinate safe transitions of care with referring acute hospitals, post-acute partners, families, and outpatient providers.

Qualifications
- MD or DO degree from an accredited medical school
- Board Certified in Internal Medicine, Family Medicine, Geriatrics, or Hospital Medicine
- Current unrestricted license to practice medicine in the state of employment
- Active DEA License
- Proficiency with EMR systems

This role requires travel to and from one or more assigned facilities. Depending on travel requirements, a valid driver’s license, current auto insurance, and an acceptable driving record may be required after 30 days of employment, with periodic motor vehicle record (MVR) checks conducted thereafter.
