Essential Duties & Responsibilities
- Daily Rounding: Conduct structured daily rounds on assigned IRF patients, performing focused assessments, monitoring rehabilitation progress, and addressing acute and subacute medical needs.
- Medical Management: Manage comorbidities and complications common in the IRF population, including post-stroke recovery, orthopedic post-op care, traumatic brain injury, cardiac conditions, and neurological disorders.
- Interdisciplinary Collaboration: Partner with physiatrists, physical therapists, occupational therapists, speech-language pathologists, case managers, and nursing staff to develop and execute coordinated, patient-centered care plans.
- Discharge Planning Support: Actively participate in discharge planning, including assessment of functional status, coordination with case management, and communication with receiving facilities or home health agencies.
- Order Management: Write and manage orders for medications, diagnostics, therapies, and consults; respond promptly to nursing and therapy team communications throughout the day.
- Urgent and Emergent Response: Evaluate and manage acute changes in patient condition, initiate appropriate interventions, and determine when escalation or transfer is warranted.
- Documentation and Compliance: Maintain thorough, timely documentation in the EMR consistent with IRF regulatory standards, including required physician/APP attestation requirements and CMS compliance guidelines.
- Patient and Family Communication: Communicate regularly with patients and families regarding medical status, goals of care, rehabilitation expectations, and discharge planning.
- Quality and Outcomes: Participate in quality improvement initiatives focused on reducing hospital transfers, improving functional outcomes, and supporting IRF performance benchmarks.
Qualifications
- Valid nurse practitioner or physician assistant license in state of operations & current board certification in your field
- Masters Degree in Nursing or Physician Assistant Studies
- Strong leadership, interpersonal, and problem-solving skills preferred
- Must possess excellent written and oral communication skills
- Must possess traits of punctuality, organizational skills, and ability to function within a team
Requirements
- This role requires travel to and from one or more assigned facilities
- If travel is required, Curana may in future determine that a valid driver’s license, current auto insurance, and an acceptable driving record are required
