Physical Therapy Asst-Long Term Care (Per Diem, Hours vary)

About the role

Under the direction of a NYS licensed Physical Therapist, the Physical Therapy Assistant (PTA) provides and documents physician ordered occupational therapy services. The PTA provides superior customer service by modeling the Unity Brand Promise and Core Values. Position located at Park Ridge LC & Wegman Family Cottages, 1555 Long Pond Rd., Rochester, NY.

Responsibilities

• Within standards of practice established for a PTA, completes and documents (within agency standard) related to patient care including visits notes, renewal orders and verbal orders.
• Includes progress or lack of goal achievement in all charting.
• Implements treatment programs individualized to meet patient needs and discharge plans: issues and provides instruction in the use of adaptive equipment.
• Confers with therapist directly, physician and/or interdisciplinary team members regarding patient response to therapy and any concerns.
• Provides PT gait and ambulation training
