Per Diem Physical Therapist, Long Term Care
About the role
The Physical Therapist is responsible for diagnosing physical impairments, creating plans of care, providing treatment, and educating patients. They coordinate treatment plans with various care providers and supervise physical therapy assistants and aides.
Responsibilities
- Provide skilled physical therapy treatment plans in accordance with physician orders, complying with New York State laws.
- Carefully coordinate treatment plans with a variety of care providers such as physicians, nurses, physical therapists, speech language pathologists, physical therapy assistants, or case managers as needed.
- Ensure continuity of therapeutic plans following discharge by designing home exercise programs, instructing patients, recommending and/or providing assistive equipment, and recommending follow-up programs.
- Supervise and direct physical therapy assistants and therapy aides in direct patient care and related activities.
- Teach and mentor physical therapy students during clinical rotations and shadowing.
- Comply with New York State Office of Professions regulations, Joint Commission standards, and Oswego Health policies.
- Maintain a clean work area, exam rooms, and supplies.
- Adhere to infection-control policies and protocols.
- Communicate any unexpected treatment outcomes or significant changes in the patient's medical status to referral sources or appropriate team members.
- Promptly report malfunctioning equipment to supervisor.
- Document all pertinent information on the patient's medical record, ensuring accuracy and completeness.
- Accurately and clearly document all procedures performed, meeting the needs of third-party payers.
- Use computer programs, including order entry, as assigned.
- Perform other duties as assigned.
Requirements
Graduate from an accredited physical therapy program. Current New York State Physical Therapist license or permit, or working toward one. Physical Therapy Experience preferred.
Qualifications
Strong organizational and interpersonal skills. Knowledge of CPT & ICD-10 coding procedures. Thorough knowledge of physical therapy principles and practices. Working knowledge of computer programs. Ability to multi-task efficiently and effectively. Ability to recognize and respond appropriately to urgent/emergent situations per protocols. Maintains professional appearance and personal conduct at all times. Adheres to infection-control policies and protocols. Complies with all HIPAA regulations. Demonstrates complete knowledge of safety policies and procedures. Participates in hospital emergency and disaster drills.
