Key Responsibilities

  • Implement treatment plans developed by the Physical Therapist to improve residents’ functional mobility and quality of life
  • Provide therapeutic exercises, gait training, transfer training, balance activities, and modalities as directed
  • Monitor resident response to treatment and report progress or changes to the supervising Physical Therapist
  • Accurately document treatment sessions, progress notes, and billing information in accordance with facility policies and regulatory requirements
  • Assist with patient evaluations as directed by the Physical Therapist
  • Educate residents, families, and caregivers on exercise programs, safety, and mobility techniques
  • Ensure compliance with Medicare Part A/B guidelines, PDPM requirements, and state/federal regulations
  • Maintain a safe and clean therapy environment
  • Collaborate with interdisciplinary team members including nursing, occupational therapy, speech therapy, and social services
  • Participate in care plan meetings and discharge planning as appropriate

Qualifications

  • Graduate of an accredited Physical Therapist Assistant program
  • Current PTA license in the state of practice
  • Previous experience in a skilled nursing facility or geriatric setting preferred
  • Knowledge of rehabilitation techniques, equipment, and safety protocols
  • Familiarity with Medicare guidelines and documentation requirements preferred
  • Strong communication and interpersonal skills
  • Ability to work effectively as part of a team

Work Environment

  • Skilled nursing / long-term care setting
  • Frequent standing, walking, bending, and assisting residents with mobility
  • May require lifting or supporting residents with appropriate equipment
  • Fast-paced environment with productivity expectations

Supervision

  • Reports to: Director of Rehabilitation and/or Licensed Physical Therapist
