Key Responsibilities
- Implement treatment plans developed by the Physical Therapist to improve residents’ functional mobility and quality of life
- Provide therapeutic exercises, gait training, transfer training, balance activities, and modalities as directed
- Monitor resident response to treatment and report progress or changes to the supervising Physical Therapist
- Accurately document treatment sessions, progress notes, and billing information in accordance with facility policies and regulatory requirements
- Assist with patient evaluations as directed by the Physical Therapist
- Educate residents, families, and caregivers on exercise programs, safety, and mobility techniques
- Ensure compliance with Medicare Part A/B guidelines, PDPM requirements, and state/federal regulations
- Maintain a safe and clean therapy environment
- Collaborate with interdisciplinary team members including nursing, occupational therapy, speech therapy, and social services
- Participate in care plan meetings and discharge planning as appropriate
Qualifications
- Graduate of an accredited Physical Therapist Assistant program
- Current PTA license in the state of practice
- Previous experience in a skilled nursing facility or geriatric setting preferred
- Knowledge of rehabilitation techniques, equipment, and safety protocols
- Familiarity with Medicare guidelines and documentation requirements preferred
- Strong communication and interpersonal skills
- Ability to work effectively as part of a team
Work Environment
- Skilled nursing / long-term care setting
- Frequent standing, walking, bending, and assisting residents with mobility
- May require lifting or supporting residents with appropriate equipment

Supervision
- Reports to: Director of Rehabilitation and/or Licensed Physical Therapist
- Works under the direct supervision of a Physical Therapist in accordance with state practice acts
