Our reimagined approach to rehabilitation connects directly to the patient experience, centered on transforming the road to recovery and helping patients regain their power. This role provides outpatient rehabilitation services in an in-home setting for the older adult population, including private residences or independent living facilities. The Physical Therapist Assistant evaluates and treats patients in their home environment, offering a flexible schedule and work setting under the supervision of a Licensed Physical Therapist.
Responsibilities
- Attends and contributes to patient care, staffing conferences, and required meetings as a representative of physical therapy, under the direction of the Physical Therapist.
- Adheres to department and patient treatment schedules, timely clinical and billing documentation requirements, and work-related meetings.
- Collaborates with the rehabilitation team to implement programs and activities consistent with patient needs and capabilities.
- Assists the Physical Therapist during tests, evaluations, and complex procedures.
- Instructs patients, families, and caregivers in physical therapy treatment skills and techniques under the supervision of the Physical Therapist.
- Maintains appropriate and timely documentation for all patients treated.
- Cleans, maintains, and reports the need for repair of equipment as necessary.
- Ensures patients and families receive high-quality, compassionate care that recognizes individual needs and rights.
- Implements the patient’s individualized treatment plan as established by the primary Physical Therapist.
- Provides individualized physical therapy treatments, including but not limited to:
- Therapeutic exercise (aerobic capacity conditioning, balance coordination, agility training, body mechanics, postural stabilization, flexibility exercises, gait and locomotion training, neuromotor development training, relaxation, strength, power, and endurance training).
- Functional training.
- Manual therapy.
- Airway clearance techniques.
- Integumentary repair and protection.
- Use of electrotherapeutic, physical agent, and mechanical modalities.
- Health and wellness programs.
- Falls risk interventions.
- Chronic disease management education.
- Performs other related duties as required.
Requirements
- Valid license in the state(s) of practice or proof of license eligibility.
- Valid driver’s license or reliable, independent transportation to and from patients’ residences.

Pay
USD $30.00 – USD $40.00 per hour, plus a $5,000 retention bonus.
