Support patient rehabilitation within a multidisciplinary team by providing individualized treatment plans, utilizing progressive treatment modalities, and ensuring a positive rehab experience for patients and families. Collaborate with staff to deliver quality care in a supportive environment.

Responsibilities

• Provide individualized treatment plans for patients

Requirements

• Graduate of an accredited Therapy program
• Licensed in the state of practice
• Previous rehabilitation or Skilled Nursing Facility (SNF) experience preferred

• Ability to work in a supportive, progressive setting
• Responsiveness to patient needs
• Strong teamwork skills
• Knowledge of various diseases and impairments
