About the Role

Support patient rehabilitation within a multidisciplinary team by developing and implementing individualized treatment plans, assisting patients with progressive modalities, and ensuring a positive rehab experience for patients and families.

Responsibilities

• Develop and implement individualized treatment plans
• Assist patients with progressive modalities
• Collaborate with team members to deliver supportive and effective therapy services

Requirements

• Graduate of an accredited therapy program
• Licensed in the state of practice
• Preferred experience in skilled nursing facility (SNF) rehabilitation
• Ability to work in a supportive environment utilizing various treatment modalities
• Compliance with background checks and licensing/certification standards
• Adherence to a drug-free workplace
