As a 142-bed Rehabilitation and Skilled Nursing Facility, we provide a comprehensive range of services—including short-term rehabilitation and long-term care—all under one roof.
Responsibilities
- Assist the licensed therapist in the overall assessment of residents
- Support the licensed therapist in performing tests, evaluations, and complex treatment procedures
- Implement resident care programs as directed by the licensed therapist
- Carry out exercise programs and train residents in exercise, ambulation, and activities of daily living according to the Resident Care Plan
- Assist patients with therapeutic exercises, functional training, and mobility activities
- Monitor patient progress and report changes in condition to the supervising Physical Therapist
- Educate patients and families on home exercise programs and injury prevention
- Prepare and maintain treatment areas and equipment in a safe and sanitary manner
- Observe, report, and chart residents’ responses to the licensed therapist
- Participate in resident, family, and staff education as needed
Requirements
- Graduate of an accredited 2-year Physical Therapist Assistant program
- Current NYS license for Physical Therapy Assistant
- Strong interpersonal and communication skills
- Ability to follow detailed instructions and work effectively in a team environment
- Physical stamina to assist residents and perform treatment activities
- Ability to function independently, demonstrate flexibility, personal integrity, and work effectively with residents, staff, families, and external agencies

Pay
$34.00 – $40.00 per hour
Schedule
Per diem - must have weekend availability
