About the role
Provides physical therapy interventions to patients under the supervision of a licensed Physical Therapist (PT). Assists in monitoring patient progress, documenting care, and maintaining records according to policy standards. Works within the scope of practice for a Physical Therapist Assistant (PTA), including co-visits with PTs and patient re-assessments every 6 visits or 30 days. Manages a caseload, delivering services based on the PT plan of care, and participates in team meetings and training programs.
Responsibilities
- Travels to patients’ homes or facilities to deliver care.
- Assists with therapeutic activities, positioning, transfers, and use of medical equipment such as wheelchairs, lifts, and orthoses.
- Provides care in diverse environments, following infection control and safety standards.
- Utilizes technology devices for documentation and communication.
- Supports patients with activities of daily living.
- Instructs family members or paraprofessionals as needed.
Qualifications
- New York State Certification as a Physical Therapist Assistant is required.
- Minimum of two years of clinical patient care experience.
- Associate’s Degree from an approved program or equivalent.
- Valid driver’s license may be required.
- Bilingual skills and basic PC skills are preferred.
