Physical Therapy Assistant - Home Care

About the role

Provides physical therapy interventions to patients under the supervision of a licensed Physical Therapist (PT). Assists in monitoring patient progress, documenting care, and maintaining records according to policy standards. Works within the scope of practice for a Physical Therapist Assistant (PTA), including co-visits with PTs and patient re-assessments every 6 visits or 30 days. Manages a caseload, delivering services based on the PT plan of care, and participates in team meetings and training programs.

Responsibilities

  • Travels to patients’ homes or facilities to deliver care.
  • Assists with therapeutic activities, positioning, transfers, and use of medical equipment such as wheelchairs, lifts, and orthoses.
  • Provides care in diverse environments, following infection control and safety standards.
  • Utilizes technology devices for documentation and communication.
  • Supports patients with activities of daily living.
  • Instructs family members or paraprofessionals as needed.

Qualifications

  • New York State Certification as a Physical Therapist Assistant is required.
  • Minimum of two years of clinical patient care experience.
  • Associate’s Degree from an approved program or equivalent.
  • Valid driver’s license may be required.
  • Bilingual skills and basic PC skills are preferred.
