Responsibilities

• Provide geriatric care services in patients' homes and senior living communities.
• Develop and implement individualized treatment plans based on patient needs.
• Assess and document patient progress and communicate findings to healthcare providers.
• Collaborate with interdisciplinary teams to ensure comprehensive patient care.
• Adhere to HIPAA regulations and maintain patient confidentiality.

Requirements

• Current license to practice physical therapy in at least one state.
• Experience in geriatric care preferred.
• Strong communication and interpersonal skills.
• Ability to travel and work in various locations.
• Pass a background check.

Qualifications

• Master's degree in Physical Therapy or related field.
• ACOTE accredited program.
• Current CPR certification.
• Knowledge of Medicare and Medicaid regulations.

Skills

• Excellent clinical judgment and problem-solving skills.
• Effective time management and organizational skills.
• Ability to work independently and as part of a team.
• Proficient in Microsoft Office Suite.
