Physical Therapy Assistant

About the Role

We are currently recruiting for a Physical Therapist Assistant to join our team at Beaver Dam Health Care Center. The purpose of this position is to treat and document care for residents with impaired ability to perform at an independent level due to disease process, under the supervision of a physical therapist. You will participate in interdisciplinary team meetings as required and help residents maintain or attain the highest level of function possible within their physical abilities.

Responsibilities

• Follow physicians’ orders for resident care and directions of the physical therapist.
• Provide appropriate treatment to help residents maintain or attain the highest level of function possible.
• Implement preventative measures and adaptive equipment to prevent decline in residents' function.
• Maintain timely and accurate resident reports as required by federal, state, and local regulatory agencies and third-party payers.
• Observe infection control procedures and ensure the safety of residents under your supervision.
• Consistently work cooperatively with residents, their representatives, facility staff, physicians, consultants, and ancillary service providers.
• Adhere to all facility policies and procedures, including Residents' Rights, Corporate Compliance Program, and safety guidelines.
• Protect the privacy and confidentiality of resident Protected Health Information.
• Attend required in-services, including those related to the Corporate Compliance Program.
• Comply with facility policies against discrimination and sexual harassment.
• Present a professional appearance and demonstrate dependability and punctuality.
• Perform all duties in an effective, timely, and professional manner.
• Accept and complete other related duties as assigned by your supervisor.

Requirements

• Associate degree from an accredited Physical Therapy Assistant Program.
• Active Physical Therapy Assistant License.

Skills

• Excellent verbal and written communication and organizational skills.
• Ability to read, analyze, and interpret common medical information.
• Ability to effectively communicate with residents, their families, and other healthcare professionals.
• Strong reasoning ability to define problems, collect data, establish facts, and draw conclusions.
