About the role
We are actively interviewing for Full-Time and PRN Physical Therapist Assistant opportunities at Aventura at Walton Hills in Walton Hills, OH. Join our in‑house rehabilitation team and help residents improve their strength, mobility, balance, endurance, independence, and overall quality of life.
Responsibilities
- Providing skilled therapy interventions under the direction of a Physical Therapist
- Implementing individualized treatment plans
- Addressing strength, mobility, balance, endurance, and functional needs
- Monitoring resident progress and response to treatment
- Educating residents, families, and caregivers on exercises and mobility strategies
- Communicating resident progress and concerns to the supervising PT
- Completing timely and accurate documentation
- Collaborating with nursing and the interdisciplinary care team
- Providing compassionate, resident‑centered care
Requirements
- Active Ohio Physical Therapist Assistant (PTA) license
- License in good standing with applicable licensing boards and regulatory agencies
- LTC/SNF, geriatric, or post‑acute experience preferred, but not required
- Net Health experience is a plus
- Strong communication, organization, and teamwork skills
- Ability to work effectively as part of a collaborative therapy team
- Passion for helping residents maintain mobility, function, and independence
