Responsibilities
- Delivers patient care according to standards, physician orders, and care plan.
- Assists in patient assessment and tracks progress within scope of practice.
- Collaborates with evaluating therapists to support treatment goals and outcomes.
- Provides education to patients, families, and caregivers according to the established plan of care.
- Supports discharge planning and goal reassessment with therapist.
- Communicates effectively with care team to optimize patient outcomes.
- Applies appropriate clinical techniques, modalities, and equipment.
- Travels to local clinics as needed.
Requirements
- Licensed (or license-eligible) Physical Therapist Assistant.
- Commitment to patient-first, integrity-driven care.
- Compassionate mindset and desire to build long-term patient relationships.

Physical Requirements
- Ability to assist patients with transfers and mobility.
