Responsibilities
- Deliver skilled therapy services directly in patients' homes.
- Implement established treatment plans focusing on therapeutic exercises, gait training, balance activities, and functional mobility interventions.
- Monitor patient progress and communicate any changes to the supervising PT.
- Encourage active participation from patients and reinforce their rehabilitation goals.
- Ensure timely and accurate documentation in Electronic Medical Records (EMR).
- Collaborate effectively with therapists, nurses, caregivers, and other members of the care team.
Requirements
- Current Colorado Physical Therapist Assistant (PTA) license in good standing.
- Graduate of an accredited Physical Therapist Assistant program.
- Valid driver’s license, reliable transportation, and auto insurance.
- Strong communication and documentation skills.
- Ability to work independently within a home-based setting.
