Responsibilities

• Deliver skilled therapy services directly in patients' homes.
• Implement established treatment plans focusing on therapeutic exercises, gait training, balance activities, and functional mobility interventions.
• Monitor patient progress and communicate any changes to the supervising PT.
• Encourage active participation from patients and reinforce their rehabilitation goals.
• Ensure timely and accurate documentation in Electronic Medical Records (EMR).
• Collaborate effectively with therapists, nurses, caregivers, and other members of the care team.

Requirements

• Current Colorado Physical Therapist Assistant (PTA) license in good standing.
• Graduate of an accredited Physical Therapist Assistant program.
• Valid driver’s license, reliable transportation, and auto insurance.
• Strong communication and documentation skills.
• Ability to work independently within a home-based setting.
