About the role
Join our team as a Physical Therapist dedicated to enhancing the quality of life for senior participants in a person-centered care program.
Responsibilities
- Assess, plan, and implement physical therapy services for participants.
- Conduct in-person evaluations at enrollment and as needed, including home safety assessments.
- Develop and administer treatment plans aligned with recognized standards.
- Educate participants and families on procedures, home programs, and safety measures.
- Train staff in body mechanics, equipment use, transfers, and mobility techniques.
- Collaborate with an interdisciplinary team to create comprehensive care plans.
- Document evaluations, treatments, and progress accurately and timely.
- Participate in team meetings, quality improvement, and organizational activities.
- Ensure compliance with policies, safety guidelines, and confidentiality standards.
Qualifications & Skills
- Advanced degree in Physical Therapy from an accredited institution.
- Current state licensure to practice physical therapy.
- Experience working in interdisciplinary teams, preferably with elderly or frail populations.
- Bilingual in Hmong preferred.
- Valid driver’s license and auto insurance.
