About the role

Join our team as a Physical Therapist dedicated to enhancing the quality of life for senior participants in a person-centered care program.

Responsibilities

• Assess, plan, and implement physical therapy services for participants.
• Conduct in-person evaluations at enrollment and as needed, including home safety assessments.
• Develop and administer treatment plans aligned with recognized standards.
• Educate participants and families on procedures, home programs, and safety measures.
• Train staff in body mechanics, equipment use, transfers, and mobility techniques.
• Collaborate with an interdisciplinary team to create comprehensive care plans.
• Document evaluations, treatments, and progress accurately and timely.
• Participate in team meetings, quality improvement, and organizational activities.
• Ensure compliance with policies, safety guidelines, and confidentiality standards.

Qualifications & Skills

• Advanced degree in Physical Therapy from an accredited institution.
• Current state licensure to practice physical therapy.
• Experience working in interdisciplinary teams, preferably with elderly or frail populations.
• Bilingual in Hmong preferred.
• Valid driver’s license and auto insurance.
