About the role

As a Physical Therapist in Home Health at Vivie, you'll be part of a supportive and flexible environment focused on enhancing the quality of life for clients.

Responsibilities

  • Evaluate clients by obtaining and interpreting subjective and objective data.
  • Develop personalized treatment plans with short-term and long-term goals in accordance with physician orders.
  • Deliver high-quality care that empowers clients to achieve their functional goals.
  • Perform re-evaluations as required to assess progress and adjust care plans accordingly.
  • Maintain accurate client records and billing using Vivie's EMR system.
  • Demonstrate a thorough understanding of all payer sources and authorization requirements.
  • Complete and document supervisory visits with PTAs and/or HHAs in compliance with regulatory guidelines.
  • Recommend and oversee continuation programs to support long-term client success and optimize discharge outcomes.
  • Make referrals to continuation programs as necessary to promote maximum independence and functionality.
  • Maintain clear and compassionate communication with clients, families, referral sources, physicians, and the interdisciplinary team.
  • Utilize exceptional verbal and written communication skills to ensure cohesive and effective care delivery.

Requirements

  • Current Physical Therapist license in the state of Minnesota.
  • CPR certification within 6 months of hire.
  • Valid driver’s license and access to an insured personal vehicle.
  • Ability to pass state mandated background check.
  • Ability to read, write, and speak English proficiently to communicate effectively with clients, families, and team members.
  • Physical capability to perform job duties.

Qualifications

  • Experience in home health care setting preferred.
  • Strong interpersonal and communication skills.
  • Knowledge of rehabilitation techniques and patient care protocols.
  • Ability to work independently and as part of a team.

Skills

  • Excellent communication skills.
  • Proficiency in computer systems, particularly electronic medical records (EMR).
  • Ability to manage multiple tasks and prioritize effectively.
  • Strong problem-solving and decision-making abilities.

Pay

$41.04/hour to $52.00/hour (hourly non-exempt) based on qualifications, experience, and location.

Schedule

PRN to part time hours (16-32 hours per pay period), Fridays, Saturdays, and Sundays.

Travel Requirements

This role requires travel.
