About the role
As a Physical Therapist in Home Health at Vivie, you'll be part of a supportive and flexible environment focused on enhancing the quality of life for clients.
Responsibilities
- Evaluate clients by obtaining and interpreting subjective and objective data.
- Develop personalized treatment plans with short-term and long-term goals in accordance with physician orders.
- Deliver high-quality care that empowers clients to achieve their functional goals.
- Perform re-evaluations as required to assess progress and adjust care plans accordingly.
- Maintain accurate client records and billing using Vivie's EMR system.
- Demonstrate a thorough understanding of all payer sources and authorization requirements.
- Complete and document supervisory visits with PTAs and/or HHAs in compliance with regulatory guidelines.
- Recommend and oversee continuation programs to support long-term client success and optimize discharge outcomes.
- Make referrals to continuation programs as necessary to promote maximum independence and functionality.
- Maintain clear and compassionate communication with clients, families, referral sources, physicians, and the interdisciplinary team.
- Utilize exceptional verbal and written communication skills to ensure cohesive and effective care delivery.
Requirements
- Current Physical Therapist license in the state of Minnesota.
- CPR certification within 6 months of hire.
- Valid driver’s license and access to an insured personal vehicle.
- Ability to pass state mandated background check.
- Ability to read, write, and speak English proficiently to communicate effectively with clients, families, and team members.
- Physical capability to perform job duties.
Qualifications
- Experience in home health care setting preferred.
- Strong interpersonal and communication skills.
- Knowledge of rehabilitation techniques and patient care protocols.
- Ability to work independently and as part of a team.
Skills

- Proficiency in computer systems, particularly electronic medical records (EMR).

Pay
$41.04/hour to $52.00/hour (hourly non-exempt) based on qualifications, experience, and location.
Schedule
PRN to part time hours (16-32 hours per pay period), Fridays, Saturdays, and Sundays.
Travel Requirements
This role requires travel.
