About the role
We are seeking a Physical Therapist to deliver patient-centered care in a home health setting.
Responsibilities
- Evaluating patients
- Developing and implementing personalized treatment plans
- Monitoring progress
- Assisting physicians with assessments
- Instructing patients and families on therapy techniques and device usage
- Ensuring safety and compliance with healthcare standards
- Documenting clinical notes
- Planning discharges
- Participating in team conferences and training
- Reporting patient reactions and changes
- Maintaining confidentiality
- Adhering to policies and emergency procedures
- Provide quality therapy services
- Support patient independence
- Collaborate with healthcare teams
- Uphold the agency’s core values of commitment, advocacy, respect, and excellence
Qualifications
- Active license to practice in Colorado
- Preferably at least one year of experience
