Well respected skilled nursing facility (SNF) is seeking a highly motivated and energetic Physical Therapist to join the team. Candidates must be willing to support a friendly, positive, and professional environment.

Responsibilities

• Provide skilled physical therapy services in a SNF setting.

Requirements

• Active Illinois Physical Therapist license.
• BLS certification (copy required).
• Valid Social Security Number and date of birth for compliance.
• Work history with no gaps exceeding 30 days.
• One professional reference covering entire work history.
• Minimum one year of experience in a skilled nursing facility.

Preferred Qualifications

• Prior travel therapy experience.
• Experience with Rehab Optima software.
