Responsibilities

• Comprehensive physical therapy evaluations
• Development of individualized plans of care
• Delivery of therapeutic interventions to improve mobility, strength, balance, endurance, and overall functional independence
• Collaboration with an interdisciplinary team on discharge planning
• Providing caregiver and family education to support safe transitions to home or lower levels of care
• Performing detailed assessments
• Leading one-on-one or small-group treatment sessions
• Documenting progress in the EMR
• Cooking closely with therapy assistants, nurses, and physicians

• Experience: 1 year

Schedule

Consistent weekday hours with the possibility of some weekend or holiday rotation to meet patient needs.
