About the role
We are seeking a Physical Therapist (PT) for a Skilled Nursing Facility (SNF) setting in Norwalk, CT.
Responsibilities
- Assess patients' physical abilities and create personalized treatment plans
- Provide therapy to enhance mobility, strength, and independence
- Collaborate with healthcare teams for comprehensive patient care
- Educate patients and families on exercises and injury prevention
- Monitor progress, document outcomes, and adjust treatments
- Implement interventions across various settings
- Participate in discharge planning and follow-up care
Requirements
- Doctorate in Physical Therapy
- Valid CT PT license
- Current BLS/CPR certification

- On-site, daytime shift
- Up to 40 hours weekly
