About the role
As a Physical Therapist in Home Health, you will assess patients' functional status, mobility, endurance, and pain to develop personalized treatment plans. Your responsibilities include providing therapy services within your scope of practice, monitoring patient progress, and adjusting interventions accordingly. You will collaborate with physicians and the healthcare team, educate patients and families about care plans, and document visits and outcomes in accordance with policies and guidelines.
Qualifications & Skills
- Valid Physical Therapy licensure in the state of practice
- Current CPR certification
- Reliable transportation or access to public transit

Pay
