Physical Therapist In Home Health Role: Responsible for assessing and evaluating patients' functional status, mobility, pain, and medical conditions to develop and implement personalized treatment plans. Collaborates with physicians and the healthcare team to provide interventions aimed at improving patient well-being, restoring function, and relieving pain. Performs patient visits, documents progress, and adjusts care plans based on patient response. Responsibilities Evaluate patient needs and assist in developing therapy plans of care.Provide treatment within legal scope, adhering to the plan of care and coordinating with team members.Monitor and report patient responses and condition changes to relevant healthcare professionals.Educate patients, families, and team members about care plans and progress.Complete evaluations, develop treatment plans, and document all patient interactions. Qualifications & Skills Current Physical Therapy license in the practice state.CPR certification and valid driver’s license with vehicle insurance or access to transportation.Ability to work effectively in diverse home environments and with all patient demographics.
