About the Role

Licensed Physical Therapist providing personalized, one-on-one care to older adults in private homes and senior living communities, with optional telehealth support.

Responsibilities

• Conduct evaluations, develop individualized treatment plans, and deliver evidence-based therapy focused on mobility, independence, and wellness.
• Document patient progress accurately within the EMR system, adhering to regulatory standards.
• Collaborate with an interdisciplinary team including PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide virtual care to enhance access and flexibility.

Qualifications & Skills

• Valid Physical Therapy license or eligibility in practice states.
• Degree from an accredited program.
• Basic EMR proficiency and CPR certification.
• Ability to meet physical demands, including lifting, repositioning, and driving.
