Physical Therapist (Sign-on Bonus!)

About the role

We are seeking a licensed Physical Therapist to provide one-on-one care to older adults in their homes or senior living communities, utilizing the Geriatric House Calls™ model. The role emphasizes autonomy, flexibility, and meaningful patient outcomes, allowing therapists to create personalized treatment schedules and incorporate telehealth for extended care.

Responsibilities

• Deliver evidence-based physical therapy to support mobility, independence, and wellness in various settings
• Conduct evaluations, develop individualized care plans, and document within the EMR system
• Collaborate with interdisciplinary teams including PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and staff to promote safety and aging in place
• Utilize telehealth as appropriate to enhance care delivery

Qualifications & Skills

• Valid Physical Therapy license and degree from an accredited program
• Proficiency with basic computer use and EMR systems
• Current CPR certification

Physical Requirements

• Ability to perform moderate physical effort
• Assist with patient transfers and reposition patients
• Drive to care locations
• Use PPE when necessary
