About the Role

Provide personalized, in-home and senior living care to older adults, with options for virtual care.

Responsibilities

• Conduct evaluations, develop individualized treatment plans, and deliver evidence-based therapy to support mobility, independence, and wellness.
• Perform one-on-one therapy sessions in various settings including private homes and senior communities.
• Document patient progress using electronic medical records in compliance with standards.
• Collaborate with an interdisciplinary team of healthcare professionals.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Utilize telehealth and virtual care to extend access and flexibility.

Requirements

• Valid Physical Therapy license or eligibility in practice state(s).
• Degree from an accredited Physical Therapy program.
• Proficiency in basic computer use and EMR systems.
• Current CPR certification.

Physical Requirements

• Ability to perform moderate physical effort.
• Assist with patient transfers and reposition patients.
• Stand/walk for extended periods and climb stairs.
• Drive between locations.
• Wear PPE as required.
