About the role
We are seeking a dedicated Physical Therapist to provide home health services.
Responsibilities
- Evaluate patients' functional status
- Develop and implement treatment plans
- Perform interventions to improve mobility, reduce pain, and maintain maximum function
- Observe and report patient responses
- Instruct patients and caregivers
- Document progress in accordance with state guidelines
- Collaborate with the healthcare team
- Adhere to the plan of care
Requirements
- Current Physical Therapy licensure in the state of practice
- CPR certification
- Valid driver's license, vehicle insurance, or access to reliable transportation
- Ability to work effectively in diverse home environments
