About the role
We are seeking a licensed Physical Therapist to provide one-on-one care to older adults in various settings, including private homes and senior living communities.
Responsibilities
- Performing evaluations
- Developing personalized treatment plans
- Delivering evidence-based therapy to enhance mobility, independence, and overall well-being
- Documenting patient progress using electronic medical records
- Collaborating with an interdisciplinary team
- Educating patients, caregivers, and staff on safety and aging-in-place strategies
- Incorporating telehealth and virtual care to offer flexibility and expanded access
Qualifications
- Valid PT license
- Degree from an accredited program
- Proficiency in EMR and basic computer skills
- Current CPR certification
- Ability to lift and reposition patients
- Ability to drive between locations
