About the Role

The physical therapist provides services to LIFE participants with physical impairments or delays, designing and implementing therapy programs in accordance with agency standards. Responsibilities include teaching families and caregivers, and performing Early Intervention Program visits as required.

Responsibilities

• Deliver physical therapy care in home, community, or LIFE settings, working with participants, their families, and caregivers.
• Design and implement individualized therapy programs for participants.
• Provide education and training to families and caregivers.
• Perform Early Intervention Program visits as approved and required.

Requirements

• Licensed by the N.Y. State Department of Education.
• Graduate of a B.S. or M.S. degree program in Physical Therapy.
• One year of supervised experience as a Physical Therapist.
• One year of experience working with the frail elderly.

Skills

• Ability to adapt physical therapy techniques to home/community settings.
• Proficiency in reading, writing, speaking, and understanding English.
• Knowledge of Physical Therapy procedures and age-related physical/mental changes.
• Strong communication skills, including approach, tone, speed, respectfulness, and non-verbal cues.
• Competency in physical assessments (e.g., height/weight, skin integrity, ADLs, bowel/bladder control).
• Technical skills for age-appropriate care (e.g., IM injection sites, range of motion, feeding skills).

Working Conditions

• Clinician delivers care in home, community, or LIFE settings.
• May involve significant travel and occasional exposure to unclean environments or temperature fluctuations.
