Position Summary

Responsible for providing services to both inpatients and outpatients, including assessment, treatment, planning, implementation and related documentation.

Position Requirements

• Educational Requirement: Graduate of accredited School of Physical Therapy.
• Licensure Requirement: Licensed as a Physical Therapist in the State of Illinois.
• Certification Requirement: Certified in CPR through the American Heart Association.
