About the role

Join a dynamic home‑health team to deliver personalized care through targeted interventions, ongoing assessments, and thoughtful discharge planning, helping patients regain strength, mobility, and independence in the comfort of their own homes.

Schedule

Shift: Friday, Saturday, and Sunday – three 10‑hour shifts.

Pay

Attractive pay: $58.00 – $60.00 per hour.

Responsibilities

• Help patients maintain, restore, and improve movement, activity, and function.
• Provide interventions, conduct re‑examinations at least every 30 days, modify interventions as necessary, and perform discharge planning.
• Other duties as assigned.

Qualifications

• Current license to practice physical therapy in the state of assignment.
• Graduate from an accredited physical therapy educational program (CAPTE accredited).
• At least one year of related physical‑therapy experience for PTs entering home health care; if less than one year, approval from regional clinical leaders and appropriate mentoring or residency program required.
• Current CPR certification.
