Physical Therapist (PT) — Skilled Nursing

Responsibilities

• Evaluate and treat patients recovering from orthopedic surgeries, neurological events, cardiac conditions, and general deconditioning
• Develop individualized treatment plans
• Collaborate with the interdisciplinary care team
• Play a direct role in helping patients return to their highest level of independence

Requirements

• Doctor of Physical Therapy (DPT) or Master’s degree from an accredited PT program
• Current or eligible for New York State PT license

Qualifications

• Passion for geriatric rehabilitation and functional outcomes
• Strong communication and teamwork skills
• New graduates are encouraged to apply — structured mentorship is provided

All rates offered are based on position, years of applicable experience and employment type. Rate ranges shown include levels for per diems with no benefits and Independent Contractors.
