Physical Therapist (PT) - Sign-on Bonus

About the Role

The Physical Therapist (PT) provides direct rehabilitation care in a skilled nursing setting, focusing on helping patients improve their functional abilities and quality of life.

Responsibilities

• Develop, assess, and modify individualized treatment plans based on physician referrals.
• Oversee and evaluate care provided by PTAs, Rehab Aides, and students.
• Maintain accurate and timely documentation using therapy software.
• Collaborate with interdisciplinary teams, patients, and families to meet care goals.
• Follow safety protocols, ensure patient rights, and adhere to legal and organizational standards.
• Participate in community health initiatives as appropriate.
• Implement treatment plans, evaluate patient progress, chart care activities, and ensure patient safety and confidentiality.

The role requires physical activity including sitting, standing, lifting, and mobility to support patient care in a compassionate, patient-centered environment.

Requirements

• Graduate from an accredited physical therapy program (BSPT, MSPT, or DPT).
• Licensure in applicable state.
• CPR certification (obtained upon hire or during orientation).
• Preferred one year of experience; new graduates are welcome.
• Proficient in Microsoft Office, excellent communication skills, and good body mechanics.
• Ability to make independent decisions, demonstrate professionalism, and work effectively within a team.
