Physical Therapist / PT - PRN

About the role

Join a passionate rehab team at Norwood Crossing, a supportive, collaborative skilled nursing facility in Chicago, Illinois. This is a PRN (Per Request) opportunity for a licensed Physical Therapist (PT).

Responsibilities

• Provide physical therapy treatments to patients as per their individualized treatment plans.
• Collaborate with interdisciplinary teams including nurses, physicians, and other therapists to ensure comprehensive patient care.
• Document all patient interactions and progress in the electronic health record system.
• Adhere to all regulatory and accreditation standards related to physical therapy practice.

Requirements

• Licensed Physical Therapist in good standing with the state of Illinois.
• At least 1 year of recent clinical experience in a healthcare setting.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.

Qualifications

• Master's degree in Physical Therapy from an accredited program.
• Current CPR certification.
• Knowledge of evidence-based practice and patient-centered care.

• Proficiency in Microsoft Office applications.
