Responsibilities

• Provide physical therapy services to patients in accordance with their individualized treatment plans.
• Collaborate with interdisciplinary teams to develop and implement patient care plans.
• Evaluate patient progress and adjust treatment plans as necessary.
• Document patient care activities and progress in medical records.

Requirements

• Licensed Physical Therapist (PT) in good standing with the state of Illinois.
• Current CPR certification.
• Experience in skilled nursing facilities preferred but not required.

Qualifications

• Strong clinical skills and knowledge of physical therapy techniques.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.

Skills

• Proficiency in using electronic health record systems.
• Knowledge of Medicare and Medicaid regulations.
