Physical Therapist (PT) — PACE Program

About the role

As a Physical Therapist in PACE, you will make a lasting impact by providing comprehensive physical therapy services to elderly participants in a community-based setting. Your work will focus on fall prevention, mobility optimization, pain management, and functional maintenance, collaborating with an interdisciplinary team including physicians, nurses, social workers, and occupational therapists.

Responsibilities

• Provide comprehensive physical therapy evaluations and treatments for PACE participants
• Collaborate with an interdisciplinary team to develop holistic care plans
• Focus on fall prevention, mobility optimization, pain management, and functional maintenance
• Work in a community-based setting with a steady and sustainable pace

Requirements

• DPT or Master's degree from an accredited PT program
• Current or eligible for Pennsylvania State PT license
• Interest in geriatric care and community-based health
• Strong assessment and communication skills
