About the role
As a Physical Therapist in PACE, you will make a lasting impact by providing comprehensive physical therapy services to elderly participants in a community-based setting. Your work will focus on fall prevention, mobility optimization, pain management, and functional maintenance, collaborating with an interdisciplinary team including physicians, nurses, social workers, and occupational therapists.
Responsibilities
- Provide comprehensive physical therapy evaluations and treatments for PACE participants
- Collaborate with an interdisciplinary team to develop holistic care plans
- Focus on fall prevention, mobility optimization, pain management, and functional maintenance
- Work in a community-based setting with a steady and sustainable pace
Requirements
- DPT or Master's degree from an accredited PT program
- Current or eligible for Pennsylvania State PT license
- Interest in geriatric care and community-based health
- Strong assessment and communication skills
