About the Role

Responsible for coordinating and delivering physical therapy services, developing individualized treatment plans, and providing direct therapy to patients.

Responsibilities

• Evaluate patients through data collection from records, interviews, and observations.
• Design and implement personalized treatment programs to promote health and achieve goals.
• Provide therapy in various settings, tailoring strategies to patients’ physical, emotional, and cognitive needs.
• Document evaluations, treatment plans, progress notes, and reports.
• Communicate effectively with patients, families, physicians, and interdisciplinary team members.
• Collaborate with team members and participate in ongoing professional development.

Requirements

• Active physical therapist license in the state of practice.
• Preferred 1-3 years of licensed physical therapy experience.
• Ability to perform essential functions with adequate visual and communication skills.
