Physical Therapist (PT) - Outpatient

Role Overview: The Physical Therapist provides outpatient rehabilitation services in-home or in assisted living settings, focusing on older adults. Responsibilities include assessing patients, developing and implementing individualized therapy programs, educating patients and caregivers, and referring to other disciplines when needed. They supervise support personnel, ensure timely documentation, and collaborate with the rehabilitation team to optimize patient care. The role involves promoting patient independence, safety, and health through various therapeutic techniques such as exercise, manual therapy, and functional training. Key Responsibilities Evaluate patients, interpret findings, and develop treatment plans.Administer therapies including therapeutic exercises, functional training, and modalities.Design and fit adaptive devices, instruct patients on their use.Monitor progress, modify plans as necessary, and perform reexaminations.Educate patients and caregivers on safety, mobility, and chronic disease management.Collaborate with the healthcare team and refer to other disciplines as needed.Maintain documentation, order supplies, and ensure a safe environment. Qualifications & Skills Valid Physical Therapy license or eligibility in practicing state(s).Master’s degree in Physical Therapy or equivalent, or Bachelor’s plus experience.Ability to work independently with reliable transportation.Strong clinical, communication, and patient education skills.
