Physical Therapist (PT) - Outpatient

About the role

The Physical Therapist provides outpatient rehabilitation services in-home or in assisted living settings, focusing on older adults.

Responsibilities

• Assess patients, interpret findings, and develop individualized therapy programs
• Administer therapies including therapeutic exercises, functional training, and modalities
• Design and fit adaptive devices; instruct patients on their use
• Monitor progress, modify plans as necessary, and perform reexaminations
• Educate patients and caregivers on safety, mobility, and chronic disease management
• Collaborate with the healthcare team and refer to other disciplines as needed
• Supervise support personnel, maintain documentation, order supplies, and ensure a safe environment

Qualifications

• Valid Physical Therapy license or eligibility in practicing state(s)
• Master’s degree in Physical Therapy or equivalent, or Bachelor’s plus experience
• Ability to work independently with reliable transportation
• Strong clinical, communication, and patient education skills
