Responsibilities
- Establishes therapy goals and formulates a treatment plan based on evaluation results.
- Supervises Physical Therapy Assistants (PTAs) according to organization policy and state regulations.
- Provides individual, family, and group therapy sessions for clients engaged in outpatient therapy.
- Participates in interdisciplinary groups for hospice patients and case conferences for home health patients.
- Participates in discharge planning.
- Initial assessment of clinical needs using standardized tools and protocols, and facilitates recommendations for treatment.
- Records all pertinent data in the patient’s medical records.
Qualifications
- Doctor of Physical Therapy degree required.
- Experience working as a physical therapist in a hospital, rehab, or ambulatory setting preferred, but not required.
- Pelvic Health experience preferred but not required.
- Knowledge of medical terminology.
- Excellent organizational skills.
- Familiarity with Electronic Medical Record systems.
- Effective communication skills.
- Teaching others how to do something.
- Excellent problem-solving, case management, and communication skills.
- Knowledge of related accreditation and certification requirements.
